Once you have an appointment scheduled at Piedmont Injury and Rehab Center, you’ll want to prepare for your first visit to either our Stockbridge or Lithonia, GA office.
How should I prepare for my appointment?
Your first appointment will include a discussion of how you were injured and an evaluation of your current range-of-motion, pain location and levels, and your baseline function.
Do I need to bring anything?
Please bring the following to your first appointment:
- A list of your medical conditions and medications

- Information regarding any examinations you have had for your current issue
- Imaging reports related to this injury
- Paperwork or contact information for your insurance claim or lawyer
Can you help me with my workers compensation documents?
Absolutely. We work with many patients who have work-related injuries. We’re happy to help you with your paperwork or necessary reports to help you obtain the care you need.
Will you release my records to my lawyer?
We can release records only if you sign a Release of Information Form, giving us permission to send your records to specific people or companies. Once you sign, we can can send the information they need.
